Submit Proof by Mail
Congratulations, you have reached the final step. You selected to submit your proof by mail. Please follow the steps below to complete your application. If you want to select another option to submit proof, click the Previous button below and choose another option.
1. You will receive by mail a prepaid envelope with your Enrollment ID. Once you receive the prepaid envelope use it to mail back required proof documentation of your eligibility.
2. Once you are ready send the envelope with the documentation to:

*Important: Please also include your enrollment number on your proof (without enrollment number we are unable to process).
Important: Proof submitted MUST be clear and legible. We are unable to process your application if proof is not legible. Remember to only send copies of proof, please do not send originals.
