The FCC requires all Lifeline subscribers to annually certified their continued eligibility of the benefit offered through their Eligible Telecommunications Carrier. The FCC has established various methods to check eligibility of Lifeline subscribers and it is encouraged that you comply annually with their requirement to certify your continued eligibility.
If you are re-certifying through a program you are required to self certify under the law of perjury that you are enrolled in a program that qualifies you for the benefit. If you are re-certifying through Income based on the Federal Poverty Guideline table applicable to your state then you are required to provide photocopied documented proof of income. For income participation you can provide a photocopy of your W2 form or 4 consecutive months of paystubs. You are also required to certify that you are the Head of Household and that you are only receiving a single Lifeline benefit from SafeLink Wireless.
